Morgan LaHolt, MD – Staff physiatrist and Brain Injury Program Medical Director
Dr. LaHolt graduated with a Doctor of Medicine degree from the University of Nebraska College of Medicine. After an internship in Family Medicine at the University of Nebraska Medical Center, he completed the physical medicine and rehabilitation residency program at the University of Missouri-Columbia. Dr. LaHolt is board certified in the subspecialty of Brain Injury Medicine and American Board of Independent Medical Examiners.
Jackie Tran, MD – Staff Physiatrist
Dr. Tran is a graduate of Rutgers New Jersey Medical School and completed a physical medicine and rehabilitation residency at Rutgers/Kessler Institute for Rehabilitation. She specializes in non-surgical management of nerve, muscle, bone, movement and pain conditions. She is also a certified pre-employment medical examiner for the Department of Transportation.
Adam T. Kafka, MD – Staff Physiatrist, Medical Director Madonna Rehabilitation Hospitals
Dr. Kafka graduated with a Doctor of Medicine degree from the University of Nebraska College of Medicine. After an internship in internal medicine at Iowa Methodist Medical Center, he completed the physical medicine and rehabilitation residency program at the University of Wisconsin. Dr. Kafka is board certified in physical medicine and rehabilitation.
Dustin Reinbold – Physical Therapist
Dustin has been practicing physical therapy since 2000. He obtained his Master of Science in Physical Therapy from Des Moines University-Osteopathic Medical Center. He specializes in orthopedics, post–surgical and sports rehabilitation, manual therapy, joint replacement and low back rehabilitation, ergonomic assessments, and work injury rehabilitation.
Jill Smith – Physical Therapist
Jill received her bachelor’s degree in Exercise Science from the University of Nebraska-Lincoln and her Master of Science in Physical Therapy from Des Moines University-Osteopathic Medical Center. Jill specializes in post-surgical and orthopedic rehabilitation, manual therapy techniques for upper and lower quarter, low back/sacroiliac rehabilitation, ergonomic assessments, and work– injury rehabilitation, Jill is Gastron and Pilates certified.
Melissa Rose — Fit For Work Assistant
Melissa is the voice of the Fit For Work Corporate Services Hotline, serving as the first point of contact since 2009. She arranges pre-employment screens, drug screens, and DOT physicals. She guides the follow-up process for on-site work injuries to return the employee back to work in a timely manner.
Katie Jones — Corporate Wellness Developer
Katie is a registered dietitian and licensed medical nutrition therapist. She received her bachelor’s degree from the University of Nebraska-Lincoln and earned her master’s degree in Nutrition and Health Sciences with an emphasis in community nutrition and a minor in agricultural leadership education communications. She is also certified in transdisciplinary childhood obesity prevention.
Caitlin Orth — Physical Therapist Assistant
Caitlin has been a licensed physical therapist assistant since her graduation from Southeast Community College in 2015. Prior to that, she graduated from the University of Nebraska-Lincoln with a bachelor’s degree in nutrition, exercise and health science. Caitlin enjoys spending time with her husband mountain biking, camping and hiking.
Shannon Schultz — Northwest Clinic Manager
Shannon Schultz is a graduate of the University of Nebraska-Lincoln with a bachelor’s degree in exercise science. She then earned her doctorate of Physical Therapy from the University of Southern California in Los Angeles, California. Shannon specializes in orthopedic and sports-related injuries with particular interest in rehabilitation of the shoulder. She also enjoys working with patients in their return-to-work or recreational goals. Shannon is married and has two children who keep her busy with activities of all sorts.
Daniel Crowe — Physical Therapist
Daniel Crowe is a board certified clinical specialist in orthopedic physical therapy. He obtained his bachelor’s in exercise science and minor in biology from Concordia University in Seward, NE. He earned his doctorate in physical therapy from Midwestern University in his home town of Glendale, AZ. He has been a physical therapist since 2013 with experiences ranging from acute care, inpatient rehabilitation to outpatient orthopedics. He enjoys working with complex medical diagnoses, orthopedic injuries of the spine and extremities, as well as sports related injuries. When he is not working he enjoys exercising outside with his wife, playing golf, watching Husker athletics and competing in sprint distance triathlons.
